don't use the site.
Using the site
You may browse this website and print or download pages for your own
personal, non-commercial use. You must not use the site in a way that is
unlawful, that could damage or impair it, or that interferes with anyone
else's use of it — including attempting to gain unauthorised access,
introducing malicious code, or sending spam through the enquiry forms.
Intellectual property
Unless stated otherwise, all content on this website — including text,
graphics, the Communio name, logo and brand elements, and the site's design —
is owned by or licensed to Communio and protected by Australian and
international copyright and trade mark laws. You may not copy, reproduce,
republish, adapt or distribute it, other than as permitted by the
Copyright Act 1968 (Cth) or with our written permission.
Links to other websites
We may link to external websites, such as government, NDIS or aged-care
resources. Those links are for convenience only; we don't control and aren't
responsible for their content, availability or privacy practices.
Availability
We aim to keep the site available and accurate, but we don't guarantee it
will be uninterrupted, error-free or free of viruses. We may change, suspend
or remove any part of the site at any time.
3. Health information disclaimer
The information on this website is general in nature. It is not
clinical advice and is not a substitute for an individual assessment by a
qualified health professional who knows your circumstances. Please don't act,
or stop acting, on anything here without getting advice specific to you.
Information about the NDIS, My Aged Care, Support at Home and other funding
schemes is a plain-English summary and may change. Always check current
details with the relevant agency or your plan manager.
This website and our inbox are not monitored for emergencies.
If you or someone else is in immediate danger, call 000.
For crisis support, call Lifeline on 13 11 14 (24/7).
4. Privacy policy
We respect your privacy. This policy explains how Communio collects, holds,
uses and discloses personal information, in line with the Privacy Act 1988
(Cth) and the Australian Privacy Principles (APPs). As a health service
provider, we are covered by the Privacy Act regardless of our size.
We also comply with [applicable state health records legislation].
What we collect
- Contact details — name, email, phone number and address.
- Referral details — who is making the referral, their relationship
to the client, and the funding type (NDIS, My Aged Care / Support at Home,
private).
- Health and support information — medical history, diagnoses,
functional abilities, goals, home environment, equipment, and information in
NDIS plans or aged-care assessments.
- Funding and billing details — NDIS number, plan-management
type, plan manager or support coordinator details, and invoicing records.
- Other people's details — such as family members, carers, GPs
and other professionals involved in your support.
Health information is "sensitive information" under the Privacy Act. We only
collect it with your consent (or your authorised representative's consent), or
where the law otherwise permits, and only when it's reasonably necessary to
provide our services.
How we collect it
Usually directly from you — through the website's referral and contact forms,
by email, by phone, or during assessments and sessions. With your consent, we
may also collect information from referrers, family members, carers, GPs,
specialists, support coordinators, plan managers, aged-care providers, the NDIA
or My Aged Care. If someone refers you to us, we'll let you know what we've
received and from whom.
Why we collect it
- to respond to enquiries and referrals;
- to assess, plan and deliver occupational therapy services;
- to write reports, recommendations and funding applications you've asked for;
- to coordinate your care with others involved in your support, with your consent;
- to issue invoices and claim payment from funding bodies or plan managers;
- to meet our legal, professional, insurance and regulatory obligations; and
- to handle feedback and complaints, and improve our services.
We don't use your information for direct marketing, and we never sell it.
Who we may share it with
Only as needed for the purposes above, and only with your consent unless the
law requires or permits otherwise. This may include: your GP and other treating
health professionals; the NDIA, your plan manager or support coordinator;
My Aged Care, Services Australia or your aged-care provider; equipment suppliers
and tradespeople (for home modifications); and our professional advisers and
insurers.
We may also disclose information without consent where required or authorised
by law — for example, to lessen or prevent a serious threat to someone's life,
health or safety, to meet mandatory reporting obligations, or in response to a
subpoena or court order.
Storage, security & overseas disclosure
We take reasonable steps to protect personal information from misuse,
interference, loss, and unauthorised access, modification or disclosure. Records
are kept in secure, access-controlled systems.
We use [practice-management software] and [email provider] to store records and
correspondence.
Some of these providers may store data on servers outside Australia. If the
website's form delivery service is enabled, form submissions are processed by
Formspree, whose servers are in the United States. Where information is held
overseas, we take reasonable steps to ensure it is handled consistently with the
Australian Privacy Principles.
How long we keep it
Health records are kept for the periods required by law — generally at least
7 years from the last service for adults, and for a person who
was under 18 at the time, until they turn 25.
After that, records are securely destroyed or de-identified.
Anonymity
You can ask general questions without identifying yourself or by using a
pseudonym. However, we can't provide clinical services or claim funding without
identifying you.
Accessing and correcting your information
You can ask to see the personal information we hold about you, or ask us to
correct it, by emailing
hello@communio.com.au.
We'll respond within 30 days. There's no charge to make a request; we may
charge a reasonable fee for copying large records, and we'll tell you first. If
we refuse access or correction, we'll explain why in writing, as the law allows.
Data breaches
If a data breach is likely to cause serious harm, we'll notify affected
individuals and the Office of the Australian Information Commissioner (OAIC) in
line with the Notifiable Data Breaches scheme.
Privacy complaints
If you have a concern about how we've handled your information, please contact
us first — we'll acknowledge it within 2 business days and aim to resolve it
within 30 days. If you're not satisfied, you can contact the
OAIC on 1300 363 992 or at
oaic.gov.au.
5. Cookies & analytics
This website does not use cookies, analytics, advertising trackers or
third-party embeds, and it doesn't load resources from other websites — fonts
and images are hosted with the site. The only information we receive through the
website is what you choose to send us through a form or email.
Our web host may keep standard server logs (such as IP address, browser type and
pages requested) for security and maintenance. If we ever introduce cookies or
analytics, we'll update this section first.
6. Service terms
Referrals & consent
Anyone can make a referral, but we'll only provide services with the informed
consent of the client or their authorised representative (such as a guardian or
nominee). You can withdraw consent at any time.
Service agreements
Before services start, we'll give you a written quote and — for NDIS
participants — a service agreement setting out the supports, prices, travel,
cancellation and payment terms. The service agreement, together with these terms,
forms our agreement with you.
Fees
- NDIS — charged at or below the price limits in the current
NDIS Pricing Arrangements and Price Limits, including any travel and
non-face-to-face time allowed under those arrangements.
- My Aged Care / Support at Home — charged as agreed with you
and your aged-care provider.
- Private clients — charged at our current private rates,
quoted in writing before any service.
Invoices are payable within [7] days.
Cancellations
Please give as much notice as you can if you need to cancel or reschedule.
For NDIS participants, short-notice cancellations and no-shows may be charged as
permitted by the current NDIS Pricing Arrangements and Price Limits and your
service agreement.
For private clients, cancellations with less than [2 business days'] notice may
incur a fee of up to [100%] of the session fee. We'll never charge a
cancellation fee without it being set out in writing first.
Reports
Reports and recommendations reflect our independent professional opinion at the
time of assessment. We can't guarantee any funding outcome — decisions rest with
the NDIA, My Aged Care, your provider or other funding bodies.
Safety
Because we visit homes and community settings, we may postpone a visit if there
is a risk to anyone's health or safety (for example, an unrestrained animal or an
infectious illness). We'll always talk it through with you and reschedule.
7. Professional standards
- Registered with the Occupational Therapy Board of Australia through
AHPRA, and bound by the Board's code of conduct and registration
standards. You can check registration on the
AHPRA public register.
- Committed to the NDIS Code of Conduct and holding a current
NDIS Worker Screening Check.
- Committed to the Aged Care Code of Conduct when delivering
aged-care services.
- Holding current professional indemnity and public liability insurance.
8. Feedback & complaints
Feedback — good or bad — helps us do better, and raising a concern will never
affect the service you receive. Please contact us by email or phone, or ask
someone (a family member, carer or advocate) to contact us for you. We'll
acknowledge your complaint within 2 business days, keep you informed, and aim to
resolve it within 30 days.
If you're not satisfied with our response, or would prefer to go straight to an
independent body, you can contact:
9. Liability & Australian Consumer Law
Our services come with guarantees that cannot be excluded under the
Australian Consumer Law. Nothing on this website or in our terms
excludes, restricts or modifies any right or remedy you have under that law or
any other law that can't be excluded.
Subject to that, and to the extent permitted by law, Communio is not liable for
any loss or damage arising from your use of this website, from reliance on its
general information, or from any website we link to. Where our liability for
breach of a consumer guarantee can lawfully be limited, it is limited to supplying
the services again or paying the cost of having them supplied again.
10. Accessibility
We want everyone to be able to use this website. It is designed to work with
keyboards, screen readers and zoom, respects reduced-motion settings, and aims to
meet the Web Content Accessibility Guidelines (WCAG) 2.2 at level AA.
If something doesn't work for you, or you'd like information in another format,
please tell us and we'll help. You can also contact us through the
National Relay Service (133 677) or with an interpreter through
TIS National (131 450) — both free.
11. Governing law & changes
These terms are governed by the laws of [State], Australia, and you submit to the
non-exclusive jurisdiction of its courts.
We may update this page from time to time — for example, when the law or our
services change. The "last updated" date at the top shows when it was last
revised. If a change significantly affects how we handle your personal
information, we'll let current clients know.
